Fees
Dr. Sticken is a fee for service provider and is also contracted Blue Cross Blue Shield (BCBS) of Arizona. However, each BCBS plan is different. It is important to check your specific benefit plan (eligibility, deductible information, and co-payment). If you have a different insurance provider,
Dr. Sticken can provide you with documentation to submit to your insurance provider to potentially receive out of network reimbursement. If you plan on utilizing this option, it would be best to check with your insurance company in order to determine specific requirements as they tend to vary with each insurance provider.
Service Fees:
Initial Evaluation/Intake (for those wanting to utilize insurance for checking benefits and data entry for billing): $175
Sexual Addiction Evaluation (60 to 180 minutes): $160-$480
Psychological Evaluation with Psychological Testing (60 to 300 minutes): $320 to $800
Individual Psychotherapy (50-60 minutes): $160
Individual Psychotherapy (45 minutes): $145
Case Management (phone calls over 15 minutes, preparation of records and/or treatment summaries, consultation with other professionals): $15
Preparation and participation in legal proceedings: $400 per hour

Good Faith Estimate
Beginning January 1, 2022 – Pursuant to the No Surprises act (HR 133, Title 45 Section 149.610), you are entitled to receive this “Good Faith Estimate” of what the charges could be for psychotherapy services provided to you by Shannon Sticken, Psy.D. While it is not possible for a psychologist to know, in advance, how many psychotherapy sessions may be necessary or appropriate for a given person, this form provides an estimate of the cost of services provided. Your total cost of services will depend upon the number of psychotherapy sessions you attend, your individual circumstances, and the type and amount of services provided to you. Therefore, estimates are based on yearly time frames. This estimate is not a contract and does not obligate you to obtain any services from this psychologist, nor does it include any services that may be recommended during treatment to you that are not identified here.
The Good Faith Estimate is not intended to serve as a recommendation for treatment or a prediction that you may need to attend a specified number of psychotherapy visits. The number of visits that are appropriate in your case, and the estimated cost for those services depends on your needs and what you agree to in consultation with this psychologist. You are entitled to disagree with any recommendations made to you concerning your treatment and you may discontinue treatment at any time. I will discuss the Good Faith Estimate with you and provide you with a written copy.
